DISH’n It Up Bonus offer valid for residential customers that have had DISH Network services for more than one year and participated in DISH’n It Up on or after February 1, 2007, through January 31, 2008. No substitutions. Eligible customers will receive a one-time account credit for each DISH’n It Up receiver upgraded (max of 2 per transaction).

*Customer may not be able to combine select offers or discounts available from DISH Network. Request form must be fully completed and must include a copy of the first month’s billing statement indicating qualifying programming purchased. Keep copies of all materials sent. Materials received become the property of DISH Network and will not be returned. One redemption form per qualifying new customer account. DISH Network is not responsible for lost, late or misdirected mail. Request must be postmarked within 60 calendar days of product activation date. Any claims postmarked after 60 calendar days of product activation date will not be processed. If terms and conditions are not met, the redemption form will not be processed. $100 Back High-Speed Internet offer valid for residential DISH Network customers who purchase high-speed Internet through DISH Network as of 3/13/07 and subscribe to qualifying programming (America’s Top 100, America’s Top 200, America’s Top 250, America’s “Everything” Pak, DishLATINO, DishLATINO Dos, DishLATINO Plus, DishLATINO Max, DishLATINO “Everything” Pak, or Great Wall TV Package), during offer period. Customers who receive a single bill from a DSL provider for both DISH Network and DSL service do not qualify for this offer. No substitutions. Eligible customers will receive a $10 DISH Network account credit for up to 10 months. Along with this redemption form please submit either a copy of your first month’s billing statement indicating DISH Network High-Speed Internet powered by WildBlue or a copy of your first month’s billing statement from DISH Network plus a copy of your DSL provider’s billing statement clearly showing DSL charges.

HD Bonus offer valid for eligible new residential customers as of December 1, 2006 through August 14, 2007, and current DISH Network customers with DishHD programming (only available with America’s Top programming packages) through August 14, 2007. No substitutions. Eligible customer will receive a $10 account credit for up to 10 months. Current customer must have been active for a minimum of 60 days.
